Certificate Reprint Request

Verification of National Dental Hygiene Certification Status and Certification Number

If you are unsure of your NDHCE™ certification status you can verify your status by contacting the FDHRC™. Certification numbers are not provided over the phone or via email. You must request a reprint certificate to receive your certification number.

Reprints of National Dental Hygiene Certificate

If you have lost your NDHCE™ Certificate and/or would like another copy, the FDHRC™ can issue another "original".

Certificate reprints are available from the FDHRC™ office for $43.50 + tax, by Credit card.

You will be required to provide:

• a completed Certificate Reprint Request Form (create or log into your account here to complete the reprint request form).
• proof of legal name change (if applicable), such as a marriage certificate, divorce paper, legal name change papers, etc. (The FDHRC™ will only provide a certificate in your name at the time you received the certificate. You may then provide proof of name change to the regulator at the time of registration.);
• your current address and the name and address you would like your certificate sent to (if different); and
• payment by credit card in the amount of $43.50 plus applicable tax.

Should you wish to have your certificate couriered additional fees will apply.

Once we receive your request and payment we will place the order for your certificate reprint and you should have it within approximately 1 week.
